Sakuya Izayoi is a main character in the series Touhou Project. The Chief Maid who serves Remilia Scarlet, the head of the Scarlet Devil Mansion. She is the only human living in the Scarlet Devil Mansion. She is obsessive and thorough surrounding her work to supporting Remilia Scarlet and Flandre Scarlet. Although her personality can be seen as perfectly elegant, she nonetheless does not always fit in the best - being considered to be "weird", and generally being ostracized by human society. She possesses the ability to manipulate time, where she's able to control the flow of time, such as speeding up time, slowing it down, erasing it, and stopping time altogether. She can also compress the flow of time to make both past and future to exist at the same time, making the same object from different timelines be in many places simultaneously. She possesses a great amount of silver throwing knives and is skilled at handling them, and with only her ability to manipulate time, she is skilled in "conjuring" knives from nowhere.

- She's loyal to Remilia Scarlet and serves her as the Head Maid without question, a servitude in which no one knows why either trust each other so much. She follows Remilia's requests even when she knows it won't work, such as catching Lily White to restore Spring in Strange and Bright Nature Deity, because that's what her mistress wants.
- Sakuya eagerly caters to all of Remilia's needs, often placing them above her own wellbeing (Foul Detective Satori, Wild and Horned Hermit).
- For instance, even after being imprisoned upon suspicion of poisoning Patchouli Knowledge, Sakuya still takes care to make sure her mistress's meal is prepared just as she prefers (FDS)
- Sakuya's loyalty to Remilia is noted by other characters, with several of them calling Sakuya [Remilia's] dog (Perfect Cherry Blossom).
- Sakuya remarks on a "love duty" which some maids apparently provide for Remilia - potentially including herself (Embodiment of Scarlet Devil, Marisa route, stage 5)
- She will also do things for Remilia which she won't even ask, like accompanying her in Imperishable Night and Immaterial and Missing Power (or trying to, in the latter case), or putting on a sleight-of-hand magic show for Remilia without warning in Curiosities of Lotus Asia.
- Whenever Remilia decides to go outside the mansion, she would accompany her, holding an umbrella for her even in the onsen bath (Wild and Horned Hermit, etc.)
- Although Sakuya embodies and prioritizes proper employee etiquette and as a result dislikes Meiling's tendency to slack off, she does worry for her, as shown in Meiling's Touhou Hisoutensoku ending where after finding Meiling sleeping on the job Sakuya takes her inside because she's worried of the possibility of Meiling ending up with a heatstroke.
- Some of Meiling's lines in Touhou Hisoutensoku also reveal that Sakuya and Meiling frequently have friendly spars and Meiling goes easy on Sakuya during these times.
